Sligo Regional Hospital holds a successful open day on “Planning your discharge from Hospital”

Sligo Regional Hospital recently hosted an Open Day on “Planning your discharge from hospital” in the front foyer of the hospital.
The Open Day event was open to all patients, families, relatives, carers, staff and visitors and the key focus point of the day was the implementation of “Home by 11” which involves planning for patients to be ready to leave the hospital by 11am on the day they are going home. The planning and development of the day was aimed at making the patient’s discharge from hospital safe and timely as this forms an integral part of HSE Policy nationally.
 
The Open Day was facilitated by Sligo Regional Hospital’s Integrated Discharge Planning Group with information and advice provided by multidisciplinary team members at the hospital, the HSE Home Care Department, Private and Voluntary Homecare Providers contracted by the HSE, Private Nursing Home Representatives and medical companies which provide equipment.
